Win/Loss Statement

As a service to our Club Coast Customers, we will provide gaming information to our customers for tax purposes. To request tax information:

1. Download and print the "Gaming History Request Form"
2. Fill out the form
3. Notarize the form (forms not notarized will not be honored)
4. Mail the completed form with proper postage to:
